Login using your HIN/SRN number; If it is CHESS it will be prefixed with an "X"; if it is Insurer Sponsored it will be prefixed by "I". You should include any leading Zeros, as as it appears on your statement; (i.e. I00012345678, or X0012345678)

Use your posted code that is on your statement. If not within Australia, your country code as per your country on your statement will be one of the following:  HKG, NAM, NZL, USA or ZAF
